Assessing Health Indicators

Description

This course develops student’s skills in holistic health assessment, focusing on various health indicators and social needs to evaluate individual health risks. Emphasis is placed on integrating these findings with each patient’s unique circumstances to prioritize needs and create effective care plans. The course also explores the impact of chronic and complex conditions on health, including illness progression, destabilization points and care transitions.
